High School Weight Room Permission Form

Please fill our this release form for parental permission for your child to attend the PMHS Weight room after school from 2:30 to 3:30.
Completion of this form acknowledges that your child is report to the High School weight room ONLY and will not be permitted in any other part of the building without direct supervision. Students found in other parts of the building are subject to disciplinary action by the High School and Middle School administrations.
